PC Supervisor Annual Report Form

Canadian Shield Regional Council requests that all Pastoral Charge Supervisors complete an annual PC Supervisor Report form.  In this report, please include details about the Sacraments provided, number of Communion services, Baptisms, etc.  We would also like to know how the pastoral charge is doing over all.  Please fill out the form below by:
